Tech-enabled infusion platform Uptiv Health announced a partnership with Lyn Health, a healthcare provider for poly-chronic patients, to merge infusion therapy with chronic care management through an offering dubbed Uptiv360.
Through the partnership, patients can undergo an onboarding process via an app, followed by an evaluation by a nurse practitioner during an infusion visit. Within 48 hours, patients are connected with a Lyn Health care team to access virtual chronic care management care, including behavioral health services.

Via Uptiv, patients can access a multispecialty care team that includes access to care coordination, care navigation, behavioral health and medication-management services.

With this partnership, we’re able to seamlessly screen for behavioral health and chronic condition management issues via our patient-centric app and connect eligible patients to a professional therapist within 48 hours. Uptiv launched in August of last year with $7.5 million in funding.
In December, the company introduced a patient-engagement application where patients can communicate with their clinical team via chat and video sessions, manage appointments, perform digital check-ins and onboarding, and access their medical history.
Via Uptiv’s app, patients can also access billing and payment services, care planning and coordination, and community and caregiver services, such as adding a caregiver to their profile.
Another tech-enabled infusion company is Local Infusion, which scored $10 million in Series A funding in November.
