Home care cancer-focused platform Reimagine Care announced it is joining forces with healthcare system MedStar Health to integrate its on-demand oncology care platform into the health system’s cancer care delivery model and assist patients with the transition from in-clinic care to the home.
Reimagine Care’s patient-centered model provides patients with customized all-inclusive care from their homes using technology, data insights and a multidisciplinary care team.
The partnership with Reimagine Care allows MedStar Health, based in the Washington, D.C., area, to extend its cancer care services beyond the clinic with additional symptom management, remote patient monitoring and supportive care.
“We believe the future of cancer care will be defined by how well we meet patients where they are, physically, emotionally and clinically,” Dan Nardi, CEO of Reimagine Care, told MobiHealthNews.
“This partnership with MedStar Health is a powerful step toward that future. It reflects what’s possible when two organizations come together with a shared belief that cancer care can be more human, more connected and more aligned with the lives people want to lead, not just the treatments they need to receive.”
Nardi added: “Just as importantly, it’s about supporting the oncologists and care teams who are being asked to do more than ever. They deserve solutions that expand their reach without adding to their burden.”
THE LARGER TREND
Nardi joined other healthcare executives for MobiHealthNews’ 2024 year-in-review series in December.
When asked how he saw AI’s place in the sector changing, Nardi said AI in healthcare will continue to gain traction in 2025 and each year after.
“In 2024, it seemed like many organizations were doing small initial phases to test the efficacy of various AI tools. Going forward, AI will become more widely used in all facets of healthcare, including day-to-day patient engagement and management, further automating processes that historically have been done manually,” Nardi said.
Last year, Reimagine Care partnered with Houston-based Memorial Hermann-Texas Medical Center to provide oncology patients with virtual and in-home care.
Through the partnership, cancer patients are provided with 24/7 access to a team of oncology specialists. Patients can also connect with a team member via SMS text, video and phone to receive oncology-focused educational content, address concerns about their symptoms or receive at-home care.
In 2023, Reimagine Care partnered with in-home care provider DispatchHealth to bring virtual-to-home care to oncology patients.
The collaboration aims to give cancer patients access to virtual care and coordinated care between Reimagine Care, DispatchHealth and the patient’s primary oncology care team.
That same year, Reimagine Care secured $25 million in a Series A funding round led by Santé Ventures, Martin Ventures and LRVHealth. CU Healthcare Innovation Fund, City of Hope, McKesson Ventures and Sable Investments participated in the round.
In 2024, MedStar Health partnered with in-home care provider DispatchHealth to provide acute care services to patients recently discharged from Medstar facilities in Washington, D.C. The partnership focuses on avoiding readmissions and visits
Additionally, MedStar Health connects patients with DispatchHealth based on clinical needs and geographic information before being discharged.
